Our Story
Nexus Health has been established since over 10 years by group of like-minded clinicians for the clinician with vision of providing quality care to improve overall health and wellbeing of the diverse population by connecting them to the highest attainable standard of healthcare.
Our Values are to reach out to the wider community with compassion, commitment, respect and dignity.
We have extensive experience in providing healthcare services to private and public sector including First Contact Physiotherapy services, NHS musculoskeletal outpatient physiotherapy services, Disability assessment for department of work and pensions, and private musculoskeletal physiotherapy services. We work closely with local GPs, Clinical Commissioning Group and Primary Care Networks.
All our healthcare professionals are registered with The Health And Care Professions Council and has extensive clinical experience.
Service Delivery
Nexus Health provides an integrated model, being patient-centred, building strong relationships with other providers. Key elements includes promotion of patient self-management and self-referral, as part of a patient-centred care approach involving shared decision making, care planning and patient-centred outcomes;
Nexus Health delivers a high quality MSK care and experience to patients and improve outcomes within available resources.
Nexus health recognises the importance of evidence-based information, and we will deliver the information about patient health to be clinically accurate, up-to-date and written in a way a patient can understand. Self-management can empower individuals with a better understanding of, and control over, their symptoms and provide them with the tools to ensure their condition is understood and accommodated by others.
Evidence-based information is essential for improving patient experience, supporting self-management and enabling shared decision-making. We understand engaging patients and the public is a key priority at both national and local level throughout the NHS. We keep in consideration that The NHS long term plan to ramp up support for people to manage their own health and recent guidelines from NHS England states that by involving people in their health and care, patient experience better quality care and improved health and wellbeing, and the system makes more efficient use of resources. This is achieved by patient education through various resources like versus arthritis and NICE Guidelines.
All our clinical staff is highly qualified and HCPC registered therapist who will give first-hand information to the patients for their education and also utilise evidence-based resource for patient education
The recruitment and retention of staff is the foundation of Nexus Health. In particular we seek out practitioners with a genuine commitment to public medicine. This commitment is central to our culture, allowing us to provide an environment where our staff will flourish. It is reinforced by staff appraisals and development plans, along with a performance management system.
This is under-pinned by a rigorous quality regime, including:
- Formal quarterly reviews on key areas of service provision
- Detailed workforce planning and establishments, infection control, internal incident management
- Proactively seeking patient feedback and
- Clinical audit and supervision
During the patient’s journey, the clinical presentation will drive the pathway and sign posts for self-management, with an emphasis on face to face consultation where appropriate, to provide best clinical and evidence-based management. Nexus Health will make use of the best available clinical resources for self-management (versus arthritis patient information leaflet) till the need required for further enhanced assessment and intervention after clearing the red flags.
We have a robust red flag screening strategy for safety-netting and on-call support for dealing with uncertainties. We signpost the patients to appropriate social prescribing or link worker within the community. This focus of the patient’s care will improve their quality of life through our services being integrated with their GP’s work and that of other healthcare professionals.
To ensure clinically safe and efficiency, inclusion and exclusion criteria including red flags and referral pathways will be clear. Various pathways will be agreed with the CCGs, PCN Directors and will be clearly demonstrated with the referral templates. Any red flags will be flagged appropriately to either accident and emergency or referred-back to the GP, along with an explanation.
Exclusion and inclusion criterion will be produced for the triage team. Options proposed by test sites include surgical referrals for T&O, MSK, Pain Management and Rheumatology (or a subset of these) referrals.
Nexus Health will ensure high delivery of quality MSK care and experience to patients and improve outcomes within available resources, through evidence-based practice and driven by NICE guidelines.
Service improvements and excellent patient experience will be facilitated through- friends and family questionnaires;
- EQ5Ds;
- Systematic analysis of compliments and complaints
- Internal service and external stake holder dashboards.
The service will be accessible to patients through virtual and telephone services. Face to face appointments will be available where appropriate – for instance for complex cases, with anxious patients or those who face difficulties communicating virtually. Patients will be pre-screened to minimise risks associated with Covid 19, in line with Government guidelines.
